Please be prompt and bring the following on your first appointment with KVPTC:

·   Updated doctor’s order

·   Copies of medical reports such as CT scans, ultra sound, X-rays, and hearing test

·   All current insurance information

·   Patient Info form and other materials sent to you in advance

·   A list of your current medications and dosages

·   Letter of Protection (LOP) for Personal Injury clients

·   Wear comfortable clothing and shoes
